CMS Updated Its Nursing-Home Quality List. Here Is How Families Can Use It.
Choosing a nursing home is often a high-stakes decision made on a short timeline. A federal update released August 2026 gives families a timely reason to slow down and check more than a brochure, tour, or star rating.
The Centers for Medicare & Medicaid Services (CMS) maintains the Special Focus Facility (SFF) program for nursing homes with a history of serious, recurring quality problems. Active Special Focus Facilities receive more frequent inspections and may face escalating enforcement while they work toward improvement. CMS also publishes a broader candidate list of facilities that qualify for possible selection into the program.
That distinction matters. Candidate status is not the same as active Special Focus Facility status. Likewise, a facility marked “Met” has satisfied the program’s criteria on its latest listed survey; it has not necessarily graduated. CMS says two consecutive qualifying surveys are required for graduation.
“A quality list should start a careful conversation, not end it,” says Lisa Hostetler Brown, Founder of LawyerLisa, LLC. “Families deserve current facts, clear answers, and a plan for what happens if care needs or conditions change.”
Families considering a facility can use the update as one part of a broader review:
Check the current facility profile, staffing information, quality measures, and inspection findings on Medicare Care Compare.
Ask the administrator what problems led to the designation and what corrective steps have been completed.
Visit in person, ideally at different times, and pay attention to staffing, cleanliness, communication, and how residents are treated.
Speak with the state survey agency or long-term-care ombudsman for additional context.
Review powers of attorney, health-care directives, payment arrangements, and the family’s plan if a move becomes necessary. Utilize an Elder Care Coordinator from a Life Care Planning Law Firm to assist with evaluating placement options based on current standings.
For South Carolina families, the August 2026 CMS posting lists one active Special Focus Facility and five candidates. Rehab Center of Cheraw appears as the active SFF, with 30 months in the program and a July 14, 2026 survey marked “Met.” Those details should be treated as dated information, not a permanent label. CMS warns that reporting delays can occur, so families should verify the latest public records before acting.
Separately, CMS will begin nationwide risk-based surveys in September 2026. CMS guidance points to a September 8th start for qualifying high-performing nursing homes. That change is meant to spend less survey time on homes that already meet strict quality criteria so states can focus more attention on complaints and weaker facilities. It is not a rewrite of the Special Focus Facility program.
No list can replace an individualized assessment. It can, however, reveal questions that deserve answers before admission and after placement. Good planning combines current quality information with clear legal authority, realistic finances, and a backup care plan.
